Option 1: Easy Registration Online
1. Select this link and register for ID·Assist Online.
2. Those registering online will be
provided with a Username and Password after
registration that allows creation of
their online record. Using the online entry method
you enter ID·Assist information directly
into the confidential and secure system.
3. To enable access to the full
ID·Assist system (online editing &
entry, printing, emailing), an ID Assist
Consent Form and Payment
Form must be completed and
faxed or mailed to:

4. After processing (less than 10 days) you
will be given full access to set-up
your ID·Assist information in
detail.
Option 2: Paper Based Entry System (we enter the
data for you - this is secure and confidential)
1. Review the Privacy Policy and Terms and Conditions.
2. Print and complete the Quick Registration
Form (adobe pdf) for
ID·Assist.
3. Print and complete the ID·Assist Consent Form (adobe pdf).
4. Include recent photos of the registrant
(prints, negative, digital file). Best results will be
obtained if a new digital photo/s is provided.
5. Print and complete the Payment
Form (adobe pdf).
6. Mail or fax this completed information
(quick registration, consent, payment forms) to:

7. We will then send the detailed
ID·Assist Registration Form for
completion. When you have completed the
ID·Assist Registration Form,
please return to ID Assist for data entry in your online
record.
