Safe Return Home ID·Assist - Launched in Victoria

In Partnership with Victoria Police

Safe Return Home - ID·Assist was launched at Victoria Police Headquarters on February 25th 2004.  The project was introduced by The Chief Commissioner of Police, Christine Nixon (Official Victoria Police press release ) and launched by Gavin Jennings  MLC, Minister for Aged Care in Victoria

The ID·Assist system is now available on the internal Police intranet throughout the State, Police mobile data terminals and also accessible via the Emergency 000 system.  All police in Victoria have immediate access to the system. Please read the Safe Return Home ID·Assist official launch press release

The program is for any person considered 'at risk' - this is someone that may become disoriented and go missing or otherwise need assistance from police/emergency services.  As such the system applies to not only older people with memory loss but others with intellectual disability, developmental delay, mental illness, physical disability or a brain injury of some type (eg. closed head injury).  Please contact Safe Return Home - ID·Assist if you have any questions on this innovative program designed to provide peace of mind and support for all Victorians.

Safe Return Home ID·Assist is now operating directly from the Victoria Police and Tasmania Police intranet.  If approved by registrants, police have immediate access to information on a person 'at risk' to help with search and identification efforts.

Medical Information

Registrants can enter Medical Management Plans, detailed medication lists and information on medical conditions in our specially developed forms. This information is stored in your own private online database area and is totally confidential.

What type of information can I store?

  1. Medical Conditions:
    Symptoms, Severity, Special requirements, Treatment plan, History, When first diagnosed, Who diagnosed, Relevant family medical history.

  2. Medications:
    Current & past medications, medication name, quantity, dosage, frequency, date started & finished.

Who will want to register with the medical information service?

  • People with complex medical conditions who need ready access to their information 24 hours / 7 days a week
  • People with chronic medical conditions who need a current and historical account of their medications, purpose, side effects and efficacy
  • Families who wish to have a consolidated list of medical information available on their children with special medical needs 
  • People travelling interstate or overseas who may need access to this vital information or provide it to medical services

How is the medical information accessed?   

The medical information is only accessed via your confidential Username and Password.  You can distribute it using the following approaches: 

  1. email directly from the system

  2. print off the information from your web area and then fax

  3. print off the information and mail
